The online form, which you can complete and submit electronically, replaces the pdf form that was previously available on this site. You will have an opportunity to preview your responses before making a final submission. After you submit your report, you will receive an e-mail confirming that your Rule 3.15 report was received. Please print or save the e-mail for your records. If you are amending a previous submission, you will have to fill out the informational section again. Note that under Rule 3.15 (D), all of the information in the report, including the mailing address, phone number, and e-mail address you provide, will be available to the public. Reports must be filed by April 15 for the preceding calendar year.
